Hours
Our office is open Monday through
Thursday from 7:30 to 5:30. See the map at the bottom of the
page for directions to our office. Parking is available on the
streets in front of and on the side of the office. Parking is
also available behind the post office.
Office Policies
Children invited. Parents by
invitation only: "We
feel strongly that the first appointment for a young child should
be the opportunity for Dr. Bill and the child to get to know
each other, to bond". Children almost always do better without
the parent present. In the event that a parent must be able to
see their children, he or she can be an "invisible observer".
Appointment Cancellation Policy:
Appointments cancelled
or broken with less than 24 hours notice may be subject to a
"broken appointment" charge of $50.00. That charge
may be increased depending on the amount of time allotted for
the scheduled appointment.
Payment policy:
Payment (cash, Visa, American
Express, Mastercard, and Discover Card) is expected when services
are rendered unless arrangements have been made in advance. As
an alternative to the above we offer Care Credit which is a convenient, low minimum
monthly payment program designed to pay for dental care and elective
treatment not covered by insurance. Depending on the program,
the amount borrowed may be interest free (see Links of Interest.)
There will be a finance
charge of 1 and 1/2% per month or 18% per annum on any unpaid
balances over 30 days. Additionally, there will be a $35.00 fee
for returned checks. In the event that that collections are needed
settle the account, attorney's fees, court costs, and interest
will be included in the amount due on any unpaid balance.
Patient With Dental Insurance:
We accept assignment from
Delta Dental only. Patients are responsible for any deductibles,
co-payments, and non-covered services at the time the service
is provided. Any balance remaining unpaid after 45 days from
the date of the service will be due in full by the patient. We
encourage the patient to check with your insurance company prior
to any treatment being performed. Please keep in mind that your
dental insurance policy is between you and your insurance company.
Patient Records:
By law, we are required
to retain all original records and x-rays. When provided with
a written request, we will provide copies of records and x-rays.
A fee, as determined by state law, will be charged.
